Responsibilities

• Assess, monitor, and document residents' wounds and skin conditions
• Provide wound care and treatments according to physician orders and established care plans
• Monitor residents for changes in condition and notify the appropriate members of the care team
• Maintain accurate and timely clinical documentation
• Collaborate with physicians, nursing leadership, and the interdisciplinary care team
• Educate residents, families, and staff on wound prevention and treatment
• Assist with developing and updating individualized wound care plans
• Promote infection prevention and ensure compliance with facility policies and applicable regulations
• Provide compassionate, resident-centered care

Qualifications

• Active North Carolina LPN or RN license
• Wound care experience required
• Long-term care experience preferred
• Strong assessment, communication, and documentation skills
• Basic leadership and supervisory skills
• Dependable, compassionate, and team-oriented
• Scheduling flexibility as needed
