Supporting the ED Transitions of Care team, the full-time Wisconsin Licensed Transition Specialist will manage member transitions in the community, coordinating care plans and conducting assessments while working remotely. Key responsibilities: Develop and oversee effective transition plans for members in collaboration with care coordination teams and providers Assist with member assessments and the transition process based on enrollment or care service needs Educate members and their families on services, requirements, and limitations related to transition planning Required qualifications: Bachelor's degree in a related field 2 to 4 years of relevant experience in healthcare or behavioral health Familiarity with Wisconsin Medicaid Experience in care coordination or management Ability to manage a high volume of inbound and outbound calls
