Supporting post-discharge members, the full-time remote Wisconsin Licensed Care Manager will assess needs, coordinate care services, and facilitate transitions for Medicaid members, primarily focusing on behavioral health treatment and support. Key responsibilities Evaluates member needs through post-discharge assessments and medication reconciliation Develops care/service plans in collaboration with interdisciplinary teams to support member transitions Facilitates education and resources to enhance member wellness and overall quality of care Required qualifications Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing Active clinical license in Wisconsin (LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN) 2-4 years of related experience in case management or behavioral health Strong knowledge of community resources and support services Experience with post-discharge care coordination
