West Virginia Licensed Nurse Navigator

To support patient care coordination, the full-time West Virginia Licensed Nurse Navigator will triage incoming patient referrals, assess needs, provide clinical dispositions, and facilitate referrals to primary and specialty care providers in a remote setting. Key responsibilities Assess and facilitate coordination of multidisciplinary care while providing communication and education in a culturally competent manner Efficiently triage new patient referrals to ensure timely access to care and collaborate with the care team on patient management Document patient interactions and maintain comprehensive medical records, serving as a conduit between patients and healthcare providers Required qualifications Current unencumbered licensure as a registered professional nurse in West Virginia or through the enhanced Nurse Licensure Compact (eNLC) Three years of registered nursing experience Bachelor of Science in Nursing Degree (BSN) preferred Certification in Basic Life Support to be obtained within 30 days of hire Experience in care coordination and outpatient/clinic settings preferred
