To support a growing integrated health center team, the full-time Florida Licensed Health Coach will provide personalized coaching and group programming to help patients manage chronic conditions and improve overall health, working in-person Monday to Thursday and remotely on Fridays. Key responsibilities Conduct individual and group coaching sessions to promote behavior change and chronic condition management Collaborate with healthcare providers to deliver chronic condition coaching based on clinical guidelines Facilitate health promotion programs and assist patients with scheduling medical appointments and referrals Required qualifications Bachelor's degree in health science, counseling, psychology, nutrition, social work, or a related field; Master's degree preferred Certification from the National Board for Health and Wellness Coaches (NBHWC) required Minimum of 2 years' experience as a health coach, including one-on-one and group coaching Strong background in health promotion and behavior change methodologies Technical expertise in Electronic Medical Record Software and Microsoft 365 applications
