Beacon Home Health is seeking an RN Case Manager to join our Home Health team on weekends. You will manage the full plan of care for hospice patients, perform visits, complete documentation, and coordinate with the IDG.
You must have one year of RN experience, an active RN license, CPR certification, and a valid driver's license with reliable vehicle access. This role requires traveling within assigned geographic area.
