Key responsibilities Process agency home health referrals accurately and in a timely manner Complete basic diagnosis coding based on referral information Coordinate care for homecare patients using the Welcome Call Script Required qualifications Associate's Degree preferred At least one year of experience in Home Health preferred Minimum one year of experience in a customer service role Experience with Homecare Homebase (HCHB) is required Availability to work every Saturday and Sunday from 6:30 AM to 5 PM EST
