Position Overview

The RN provides and directs nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies.

• Coordinate the total plan of care with other health care professionals and maintain continuity of patient care.
• Conduct initial and comprehensive nursing evaluation visits, determine primary focus of care, develop the plan of care within state-specific guidelines, and ensure eligibility and medical necessity.
• Maintain and submit accurate documentation and clinical notes, including patient condition, goals, interventions, and responses.
• Provide high quality clinical services within the scope of practice as defined by state laws, from admission through discharge.

Must Haves

• One (1) year of clinical RN experience.
• Current RN license in the state where services are rendered.
• Current CPR certification, driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Ability to reliably commute to multiple patient residences daily within the geographic area assigned.
