To support a growing interdisciplinary care team, the full-time Washington State RN Case Manager will assess patient health statuses, establish care goals, and collaborate with healthcare professionals remotely to address medical, functional, and social barriers to care. Key responsibilities: Assess the health status of members and establish goals to meet identified healthcare needs Plan, implement, and evaluate responses to the plan of care while collaborating with the multidisciplinary team Provide counseling and education to patients and families regarding health maintenance and disease prevention Required qualifications: Current unrestricted licensure as an RN in Washington 2+ years of experience as an RN Experience in assessing the medical needs of patients with complex behavioral, social, and/or functional needs Proficiency in using Electronic Medical Records Access to reliable home internet and a home office to maintain HIPAA privacy
