Providing nursing care to a defined group of patients, the full-time salaried RN Call Center Advice Line Nurse will work remotely from Washington or Idaho, coordinating health care services and facilitating patient assessments and care plans in collaboration with the health care team. Key responsibilities: Conduct ongoing assessments and develop individualized care plans based on patient needs Provide telephone consultations and education to patients and their families regarding health care goals and interventions Collaborate with health care team members to ensure continuity of care and address psychosocial, cultural, and emotional needs of patients Required qualifications: Associates degree in Nursing from an accredited program Registered Nurse License (Washington) or Compact License required at hire Basic Life Support certification required at hire Experience in ambulatory care nursing preferred Bachelor of Science in Nursing (BSN) is a plus
