Providing comprehensive nursing care remotely, the full-time Washington State Licensed RN Nurse will assess patient needs, develop care plans, and coordinate health services for a defined group of patients while adhering to established nursing standards. Key responsibilities: Conduct ongoing assessments of patients' nursing care needs and develop individualized care plans Provide telephone consultations and education to patients and their families regarding health care goals and interventions Collaborate with the health care team to modify care plans based on patient responses and needs Required qualifications: Associates degree in Nursing from an accredited program Registered Nurse License (Washington) or Compact License required at hire Basic Life Support certification required at hire Experience in ambulatory care nursing is preferred Bachelor of Science in Nursing (BSN) is preferred
