Key responsibilities: Conduct ongoing assessments of patient care needs and develop individualized care plans Provide telephone consultations and education to patients and their families regarding health care goals and interventions Collaborate with healthcare team members to ensure comprehensive patient care and modify care plans as needed Required qualifications: Associates degree in Nursing from an accredited program Registered Nurse License in Washington or a Compact License required at hire Basic Life Support certification required at hire Experience in ambulatory care nursing is preferred Bachelor of Science in Nursing (BSN) is preferred
