To support Medicaid members who are pregnant or recently pregnant, the remote Washington RN Maternal Case Manager will engage with clients to conduct comprehensive needs assessments, develop person-centered care plans, and coordinate care while advocating for members' health needs. Key responsibilities: Engage members telephonically to complete comprehensive needs assessments covering medical, behavioral, and socioeconomic factors Develop and implement care plans addressing chronic health conditions and promote wellness in alignment with evidence-based guidelines Collaborate with internal care teams and community resources to ensure effective implementation of care plans and support member education Required qualifications: Current, unrestricted RN licensure in the state of Washington 2+ years of maternal clinical experience 1+ years of experience with MS Office applications, including Word, Excel, Outlook, and Teams Designated quiet workspace and access to secure high-speed internet via cable/DSL Residency in the state of Washington
