Washington Licensed Social Worker

To support a growing multidisciplinary care team, the full-time Washington Licensed Social Worker will provide remote behavioral health assessments, brief therapy, crisis intervention, and care coordination for patients with complex needs. Key responsibilities Support the behavioral health needs of an assigned patient caseload using evidence-based models of care Collaborate with prescribing providers on psychotropic medication management and treatment adherence Document patient encounters and treatment plans in the electronic health record accurately and timely Required qualifications Master's degree in Social Work Current Washington independent licensure required 2+ years of prior healthcare or behavioral health experience Geriatric care experience highly desired Proficient in Microsoft Office Suite and electronic medical records (EMR) systems
