To support families affected by chronic conditions and serious illness related to aging, the part-time Washington Licensed Care Navigator will provide virtual biopsychosocial support, personalized care planning, and community resource connections while collaborating with a transdisciplinary clinical team. Key responsibilities Act as the primary point of contact for caregivers and families, enhancing their quality of life and safety at home Conduct biopsychosocial assessments to identify needs and develop personalized care plans Communicate with healthcare specialists to ensure continuity of care and provide caregiver education Required qualifications Two years of experience in a healthcare or community-based setting preferred One year of experience understanding the needs of family caregivers and older adults Familiarity with Electronic Health Records (EHRs) is a plus Strong computer skills and ability to troubleshoot technology issues Proficiency in another language, other than English, is a plus but not required
