Providing nursing care to a defined group of patients, the full-time Washington Licensed Call Center RN will assess patient needs, develop care plans, and coordinate health care services remotely while adhering to established nursing standards. Key responsibilities: Conduct ongoing assessments of nursing care needs and develop individualized care plans Provide telephone consultations and education to patients and families regarding health care goals and interventions Collaborate with the health care team to modify care plans based on patient status and needs Required qualifications: Associates degree in Nursing from an accredited program Registered Nurse License (Washington) or Compact License required at hire Basic Life Support certification required at hire Experience in ambulatory care nursing or case management preferred Bachelor of Science in Nursing (BSN) is a plus
