About This Role
The VNA RN Case Manager is responsible for providing skilled care and case management to patients and families in accordance with the State Nurse Practice Act and accepted professional standards of practice. This role is ideal for nurses who are passionate about home care and hospice services.

What You'll Do
- Provide skilled care and case management to patients and families.
- Communicate and document observations and assessments.
- Promote and maintain health through direct care, teaching, and counseling.
- Operate within established organizational and departmental policies and procedures.
Requirements
Education:
- Other Certificate/Diploma in Nursing required, Bachelor's Degree in Nursing preferred
Experience:
- Minimum of one (1) year of experience in a home care/hospice setting required
- Hospice experience preferred
Licenses & Certifications:
- Current RN License in (State)
- Class D Passenger Vehicle Driver's License required
