Nurse Case Manager - Wound Care
Pooler, GA, and Hilton Head Island, SC
We are seeking a Registered Nurse to service our Wound Care patients throughout the Savannah, GA/Hilton Head, SC area. The Nurse Case Manager will travel to patients’ homes, long‑term care facilities, and other locations to evaluate patients and initiate telemedicine visits. The agenda includes facilitating care with the appropriate provider and ensuring high‑quality health care within applicable laws, regulations, and policies.
Full‑time salary‑based position. Shifts are 8‑hour (Monday‑Friday 8:00 AM–5:00 PM) or 10‑hour (Monday‑Thursday 7:00 AM–6:00 PM). Reliable transportation is required daily.

Responsibilities
- Facilitate receipt of all medical records from primary providers and specialists.
- Review medical records.
- Complete patient consents.
- Enroll eligible patients in care management.
- Initiate a care management plan of care.
- Capture diagnoses at the highest specificity and ensure acceptance.
- Complete annual wellness visits for review by the provider.
- Conduct cognitive impairment screenings.
- Perform social determinants of health assessments and/or screenings.
- Document advanced care planning discussions with patients and provider.
- Evaluate needs for home health, hospice, palliative care, or specialty consults.
- Assess potential use of remote patient monitoring devices or resources.
- Coordinate and communicate care.
- Reconcile prescribed and OTC medications, vitamins, supplements, herbal remedies, and other treatments.
- Provide post‑discharge education.
- Recommend adaptive equipment and durable medical equipment.
- Ensure safe environment.
- Evaluate acute conditions or follow‑up from previous visit.
- Accurately document care management activities and maintain up‑to‑date care plans.
Qualifications
- Registered nurse licensed and in good standing; RN preferred.
- Minimum three years of clinical experience preferred.
- Experience in community settings preferred.
- Strong communication and collaboration skills with interdisciplinary care teams, patients, and caregivers.
- Excellent written and verbal communication.
- Basic computer literacy.
- Demonstrated leadership and effective management abilities.
- Good interpersonal skills in all conditions.
- Team player who can manage multiple responsibilities and exercise sound judgment.
- Flexible work schedule and travel between offices and facilities.
- Licensed driver with an insured, well‑maintained vehicle.
