To support innovative healthcare delivery, the full-time Virtual Hospitalist APP will conduct virtual assessments, manage care coordination, and collaborate with clinical teams remotely to provide hospital-level care to patients eligible for in-home hospitalization. Key Responsibilities Conduct virtual assessments and develop multidisciplinary care plans for adult patients with acute and chronic conditions Screen incoming referrals and assess patient eligibility for Hospital Alternative Care while leading onboarding workflows Serve as a consultative resource for field teams, guiding care plans and optimizing patient flow through collaboration Required Qualifications Master's or Doctoral degree in Nursing or Physician Assistant Studies Current unrestricted licensure and national board certification (NP or PA) Minimum 5 years of hospitalist or acute care experience Experience in virtual care or telehealth preferred DEA registration and prescriptive authority
