Virtual Family Nurse Practitioner, NY License Required

• Review the daily caseload and prepare technology and tools for patient care


• Navigate the daily schedule using modern technology


• Partner with clinical in-home team members visiting patients’ homes


• Build welcoming connections with diverse patients


• Conduct virtual adult and/or pediatric wellness visits


• Perform health risk assessments, medication reviews, and cognitive screenings


• Provide patients with educational information about health and well-being


• Collaborate with Sprinters to evaluate vital signs and arrange blood draws


• Coordinate preventive care interventions, medication management, specialist referrals, community resources, and other patient services


• Document clinical findings and provide comprehensive care coordination and management

Requirements

• Board Certified as a Family Nurse Practitioner


• Active Family Nurse Practitioner License in New York


• Prior experience performing wellness visits


• High professionalism, integrity, accountability, confidentiality, care, and compassion


• Willingness to solve problems in a rapidly changing environment


• Technological proficiency with laptops or mobile devices for charting


• Comfortable using HIPAA-secure messaging apps for care coordination


• Strong written and verbal communication skills


• Ability to work independently or in a team environment


• Must pass a national background check and valid clinical license search


• Must hold prescriptive authority in the state(s) where licensed to practice as a Nurse Practitioner
