About the Role
Circle Health is looking for compassionate and detail-oriented Virtual Care Coordinators to support physician partners and senior-care practices across Florida.
As part of the Chronic Care Management (CCM) team, you will help patients with long-term conditions such as diabetes, hypertension, and heart disease remain connected, supported, and compliant with their care plans.
This role involves monthly patient outreach, care coordination, documentation, and communication between patients, nurses, and healthcare providers.
This is a fully remote role open to candidates residing anywhere in the United States, with training and clinical supervision provided.
Key Responsibilities
Conduct structured monthly check-ins with CCM patients via phone
Review medications, vitals, and health goals while updating EMR records accurately
Track preventive screenings, labs, and follow-up activities
Coordinate between patients, nurses, and providers for appointments and care gaps
Maintain accurate documentation as per CMS CCM guidelines
Log activities and time spent for compliance and reporting purposes
Identify patient concerns or clinical red flags and escalate appropriately
Requirements
Must currently reside in the United States
1 3 years of experience in healthcare coordination, telehealth, medical assisting, or related healthcare roles preferred
Experience in CCM is an added advantage
Strong verbal and written communication skills in English and Spanish
Familiarity with EMRs such as Elation, Athena, PCC, or similar platforms
Compassionate communication style and comfort working with older adults
Understanding of HIPAA and patient confidentiality standards
Associate degree or certification as Medical Assistant, CNA, or LPN preferred
