What you need
- Active, unrestricted Nurse Practitioner license in the state where you will work
- Board certification
- NPI number
- Current ACLS and BLS
- Active DEA
- Your own office within 50 miles of Murray, UT
- CV in month and year format
- Send your CV, state license, board certification, ACLS and BLS, DEA, and NPI.
- Once you are accepted, the rest of the credentialing paperwork is due within 3 days.
- Online training runs about 8 to 10 hours, is paid, and must be finished within 7 business days.
- You then complete a virtual pre-exam orientation, and your first exams are scheduled.
