To support a dynamic Clinical Engagement Services Center, the remote Vermont Licensed MTM Pharmacist will perform telephonic consultations, conduct comprehensive medication reviews, and ensure adherence to clinical guidelines while working on a contract basis with potential for full-time employment. Key responsibilities: Engage members telephonically to schedule and conduct pharmacist consultation appointments Utilize prescription claims data to provide clinical recommendations and assess medication regimens Document patient care notes and participate in departmental meetings to enhance clinical programs Required qualifications: Current and unrestricted Pharmacist license in the state of residence Strong knowledge of disease state management guidelines and MTM services 1+ years of experience working remotely within a cross-functional team environment Intermediate proficiency with Microsoft Word, Outlook, and Excel Bachelor's Degree or Pharm D
