You will work within outpatient clinics on a collaborative care team with physicians, NPs, PAs, MAs, and nurses to deliver longitudinal care and build trusted patient relationships.
You will conduct visits, coordinate care with specialists, schedule wellness visits, and participate in population health initiatives to improve outcomes and reduce hospitalizations.
