Value-Based Primary Care NP — Lead Patient Wellness

Oak Street Health seeks a Nurse Practitioner to deliver value-based primary care to adults on Medicare in outpatient clinics. You will see patients independently, collaborate with physicians, and provide longitudinal care within a collaborative care team, including MAs, a scribe, nurses, and care managers.

You will participate in care coordination, annual wellness visits, and health risk assessments, with a focus on documentation accuracy and patient engagement in a mission-driven environment.
