To support high-quality patient care, the part-time Utah Licensed Pharmacy Technician will obtain and review post-discharge medical records, navigate multiple electronic medical record systems, and conduct outreach to members to update medication lists, all while working remotely in the MST time zone. Key responsibilities: Obtain and review medical records related to recent discharges Conduct outreach to members and providers to confirm care relationships and medication lists Identify medication discrepancies and prepare detailed reports for pharmacist review Required qualifications: High School Diploma/GED Current, clear, and unrestricted Pharmacy Technician licensure in the state of Utah National Pharmacy Technician Certification 1+ years of retail pharmacy experience Intermediate proficiency in Microsoft Office suite, including Outlook, Teams, and Excel
