URGENTLY Hospice Registered Nurse

Job Duties:

1. Coordinate hospice care with team members, patients, and families to meet needs.
2. Evaluate the effectiveness of hospice services regularly.
3. Perform patient evaluations and informational visits as needed.
4. Use the nursing process to provide professional care.
5. Consult with physicians and the Hospice Medical Director on patient condition changes.
6. Coordinate patient services with IDG and contracted providers.
7. Submit accurate clinical notes and weekly visit schedules promptly.
8. Participate in IDG and case conferences.
9. Supervise LPNs and ensure continuity of care across services.
10. Engage in staff development meetings.
11. Prepare care plans for Hospice Aide/Home Attendant.

Requirements:

• Graduate from an accredited School of Nursing.
• Licensed as a Registered Nurse in Virginia.
• At least 2 years of hospice/home care experience, including 1 year supervisory.
• Strong assessment and problem-solving skills.
• Organizational and time management skills, knowledge of case management.
• Valid driver’s license with insured vehicle.
• Excellent verbal and written communication skills.
