Description
Unit Charge, Critical Care Stepdown LTAC – RN, Full Time (36 hours) – 7:00‑7:30 pm shift.
Responsible for providing high‑quality nursing care to a specific patient population, applying the Synergy Model for Care, and serving as a resource to colleagues while advocating for patients and families. Duties include:
- Consistently, effectively and respectfully communicating with patients, their families, and other health‑care team members while delivering safe, dependable and effective care using the nursing process.
- Demonstrating developed assessment skills and recognizing the impact of nursing care on patient outcomes.
- Documenting the nursing process accurately in the appropriate format (e.g., computer, clinical pathway, chart) and independently planning and implementing patient care.
- Utilizing patient/family input and goals in care planning while managing time effectively, incorporating the AACN Synergy Model into clinical practice, revising policies and procedures based on evidence‑based research, and evaluating documentation effectiveness.
- Achieving points and contact hours as defined in the PLAN (clinical lattice) point system to maintain position.
Minimum Qualifications
- Education: Graduate of an accredited pre‑licensure Bachelor of Science in Nursing (BSN).
- Experience: At least 2 years of recent nursing experience, with a minimum of 2 years in the area of concentration.
- Licensure: A valid, unencumbered Registered Nursing License approved by the Georgia Board of Nursing.
- Certification: Basic Life Support (BLS) certification; ACLS may be required for certain departments post‑hire.
- Additional certifications may be required based on department and specialty.
- Must meet all Clinical Nurse Employee Commitments.
- Achieve electives/points and contact hours as defined in the PLAN (clinical lattice) point system to maintain position.
- Must complete UCN/SNM criteria requirements within nine (9) months of promotion or hire, including completion of Enhancing Your Communication HLC#6488, formal preceptor training workshop, and enrollment in LEAP course.
- Preferred Qualifications: MSN or DNP degree.
