To improve medication adherence rates among patients, the full-time TX Licensed Clinical Pharmacist will lead HEDIS quality measure initiatives, conduct medication therapy management, and analyze data to enhance health outcomes in a remote setting. Key responsibilities Lead efforts to enhance HEDIS medication adherence and other pharmacy quality measures Conduct medication therapy management sessions to address barriers and educate patients Monitor HEDIS performance metrics and generate reports to assess intervention effectiveness Required qualifications Doctor of Pharmacy (PharmD) degree from an accredited school of pharmacy Active pharmacist license in TX or other specified states Minimum of 2 years of experience in HEDIS-related medication adherence work Strong knowledge of HEDIS measures related to medication adherence Proficiency in using electronic health records (EHR) and data analysis tools
