- Provide after-hours triage, clinical assessment, and symptom management via phone.
- Coordinate care with physicians, facilities, and internal teams.
- Accurately document all interactions and care plans in EMR.
Qualifications:
- Active RN license with no current or pending restrictions.
- Hospice experience and Home Care Home Base (HCHB) experience required.
- Medical insurance with employer contribution.
