Current, valactive, unrestricted Speech-Language Pathology (SLP) state licensure BLS certification (American Heart Association) that not not expire prior to start 1 year of relical experience required, preferably in a clinic setting Experience with EPIC EMR/charting system Provide date of birth and last 4 digits of SSN for consideration License required if applicable Additional Information

Conduct initial and ongoing patient assessments, including OASIS Assessments at appropriate time points Develop and implement individualized speech therapy plans of care Communicate with physicians before start of care and as needed throughout treatment Provide education and instruction to patients, families, and care team members Accurately document care and submit required paperwork in a timely manner to maintain patient medical records Returning employees must be separated from any Intermountain facility for at least one year before becoming eligible for a travel assignment
