Required Qualifications: 1 year of skilled home health experience; Oregon State license; BCLS certification; reliable transportation; COVID-19 vaccination with booster and Oasis experience preferred
Preferred Qualifications: Experience with Oasis documentation; home health therapy experience; familiarity with EPIC and Rover charting systems
Additional Information:
Patient care may involve visits to up to six patients per day, with typical patient ratios of 1:1
No on-call required; travel within approximately 60 miles daily, minimizing drive time
Must have own transportation, with occasional maximum radius of roughly 60 miles per day
Facility supports a block schedule and considers local candidates within a 50-75 mile radius at the same rate
No weekends required; no float between units unless specified
Attire is business casual, with no specific scrubs or color mandated
Charting will be performed using system applications such as EPIC, Rover, and Oasis
