Required Qualifications: Valid state licensure as a Physical Therapist (PT), graduation from an accredited physical therapy program, at least 2 years of experience working in a SNF or similar setting, proficiency with PointClickCare electronic medical records system, ability to work in the United States, and excellent communication skills
Preferred Qualifications: Experience with Geriatric populations and interdisciplinary team collaboration
Additional Information: The role involves direct patient care, development of treatment plans, and documentation in compliance with facility protocols and regulatory standards. Candidates should be adaptable and dedicated to quality patient outcomes.
