Requirements
- Active Iowa RN license or Compact RN license required
- Minimum 3 years of RN experience required
- At least 2 years of experience performing OASIS assessments and care planning for in-home patients required
Responsibilities
- Home health setting serving patients in their homes across Plymouth County, Iowa
- Patient care involves OASIS assessments and individualized care planning for in-home patients
- Scheduled shifts are days, Monday through Friday
- One in four call rotation covering nights, weekends, and holidays
- 32–40 hours per week
- Epic EMR experience is preferred
- Responsible for the comprehensive nursing care of patients in their home environment
- Participates in the department\'s on-call rotation for nights, weekends, and holidays (one in four)
