Requirements
- Active RN license in Colorado or active NLC compact license
- Current BLS/BCLS certification
- Valid driver's license, reliable transportation, and auto insurance required
- Strong assessment and critical thinking skills
- Comfort with documentation and EMR systems, HCHB experience preferred
- Home health experience preferred but not required
- Ability to work independently and manage a structured caseload
Responsibilities
- In-person home health position providing skilled, patient-centered care in the community
- Monday-Friday day schedule with a flexible day-to-day workflow
- Includes participation in an on-call rotation
- Manageable caseloads with one-on-one patient care
- EMR documentation required, with HCHB experience preferred
- Coordinates care with LPNs, therapists, caregivers, and providers
- Perform comprehensive patient assessments and develop individualized plans of care
- Provide skilled nursing care and appropriate clinical interventions
- Monitor patient progress and communicate changes to the care team
- Educate patients and families on medications, treatments, and care plans
- Complete timely and accurate documentation in the EMR system
- Work within a supportive, interdisciplinary team
- Independent visits conducted in patients' homes
- Ideal candidates are dependable, steady, adaptable, and proactive communicators
