Position: Case Management RN - inpatient discharge planning
Schedule: Monday-Friday, 8:00 AM-4:30 PM (7.5-hour shifts with a one-hour lunch); weekends and holidays rare
Experience: 1 year inpatient case management; prior critical care preferred
Required: NY RN license, BSN, BLS, plus PRI and SCREEN certifications
Preferred: CarePort, MCG or InterQual knowledge and utilization review experience
EMR: Allscripts
Unit: Med-Surg and Telemetry floors of 33-40 patients, shared with a social worker; caseload 17-25, must handle at least 20
Duties: Discharge planning and referrals - discharge to home, community or SNF, housing and shelter packets, hemodialysis packets and KCI wound-vac referrals; round with physicians, work independently on the unit and collaborate with the interdisciplinary team and patients
Note: Utilization review is a separate department; business casual or any scrub color except royal blue or maroon, lab coat provided; 2 days hospital orientation plus 1-2 weeks with a preceptor
