American Traveler is hiring an RN Case Manager for an acute care hospital, requiring a NJ or compact RN license and prior inpatient discharge planning experience.
Requirements
- Active NJ or compact RN license required
- BSN required
- CCMC certification preferred
- Minimum 2 years of acute inpatient hospital discharge planning experience required
- Hospital experience required, including discharge planning and placement in rehabilitation facilities
- Experience with subacute patients required
- Case Management and Managed Care experience required
- In-Patient Case Management experience required
Responsibilities
- Hospital-based Care Management unit
- Coordinates patients through acute care hospitalization
- Manages all aspects of discharge planning and transition process
- Performs utilization management and quality screenings
- Works with subacute patients and discharge placement to rehabilitation facilities
- AM shift, 8:00 AM to 4:00 PM, 37.5 hours per week
- Rotating weekends required every 4th or 5th weekend
- One summer and one winter holiday coverage requiredMay float to a sister hospital as needed
- No on-call requirements
- Uses EPIC for charting
- Coordinates comprehensive discharge planning and transition of care for acute hospital patients
- Performs utilization review and quality screening as part of daily responsibilities
- Local candidates accepted
- Travelers may extend but are limited to one calendar year on assignment before requiring a break
