American Traveler is hiring a Pediatric Case Management RN with in-patient case management experience and at least 2 years of RN experience for a day-shift role with no weekends, holidays, or on-call.
Requirements
- Active RN license (MD or Compact) required
- Minimum 2 years of RN experience required
- BLS certification required
- Current in-patient case management experience required
- Experience with post-acute facility referral platforms such as NaviHealth or AllScripts required
- Pediatric experience not required, but case management and discharge planning experience is required
- Bedside experience preferred, ideally 5+ years
- ICU experience preferred but not required
- Epic EMR experience preferred
- Case management certification preferred
- Experience at a comparable facility size (500+ bed count) required
Responsibilities
- Pediatric Case Management position at a main hospital facility
- Will float to all pediatric units including NICU, PICU/PCICU, Onc, and 9N/S and 10N/S units (no float to psych)
- Day shift schedule, 8:30 AM to 5:00 PM, Monday through Friday
- 40 hours per week with no weekends or holiday coverage required
- No on-call responsibilities
- Facilitate discharge planning and coordinate post-acute care referrals for pediatric patients
- Collaborate with multidisciplinary care teams across various pediatric units
- Local candidates accepted at the same pay rate
