Responsibilities

• Perform comprehensive patient assessments and develop individualized plans of care within an ambulatory/outpatient setting
• Administer medications and treatments safely and accurately per provider orders and policy; monitor patient responses
• Recognize changes in patient condition and initiate appropriate nursing interventions or elevate to a licensed provider as needed
• Coordinate care with providers and the interdisciplinary team; facilitate referrals and follow-up to ensure continuity of services
• Document assessments, interventions, and outcomes accurately in the electronic health record; provide patient and family education about care, medications, and discharge needs
• Deliver culturally responsive, patient-centered care and contribute to infection control and regulatory compliance in a fast-paced outpatient environment

Requirements

• Current full, unrestricted RN license in Alaska or another U.S. state; Direct Hire must have applied for an Alaska nursing license before start date; Agency staff must have an active Alaska license
• BLS certification; Case Management certification preferred
• Graduate of an accredited school of nursing; Bachelor's degree preferred
• Ambulatory/outpatient care nursing experience; Case management experience preferred
• Ability to work in an ambulatory/outpatient care setting and apply independent nursing judgment aligned with Alaska BON guidelines
• Strong documentation skills and ability to collaborate with providers and interdisciplinary teams
