Responsibilities

• Plan, organize and direct home care services to meet patient needs in their homes
• Manage a patient caseload in an effective and efficient manner
• Provide patient care independently, while collaborating with other core disciplines to meet the patient’s plan or care goals
• Function as a resource to patients, family members, and other care team members
• Document care activities and communicate with the care team to support goals and outcomes
• Coordinate with multidisciplinary colleagues to ensure alignment with the patient’s plan of care

Requirements

• Active Registered Nurse license in California
• Experience in case management or home health settings preferred
• Ability to manage a patient caseload and coordinate care across disciplines
• Strong communication skills with patients, families and the care team
• Ability to work independently in a home care environment and adhere to care plans
