Contract | Days | Full-Time | 5x8-Hour Shifts (8:00–16:30) | Onsite / Field-Based Key Responsibilities
Provide skilled physical therapy services to home health patients in Medford, OR and surrounding areas Conduct patient evaluations and develop individualized treatment plans in a 1:1 care setting Visit up to 6 patients per day, meeting a standard productivity expectation of 30 points per day Complete clinical documentation using EPIC and OASIS Coordinate patient groupings to minimize drive time, with occasional travel radius of 50+ miles Required Qualifications
Minimum of 1 year of skilled home health physical therapy experience required Active or pending Physical Therapist license in the state of Oregon Basic Cardiac Life Support (BCLS) certification required OASIS documentation experience required Reliable personal transportation required COVID-19 primary vaccination series plus one booster required Preferred Experience / Skills
Familiarity with EPIC electronic health records system Experience meeting productivity point expectations in a home health environment Comfort with variable daily scheduling and independent field-based work Why Join Us
