Physical Therapist (PT)

Pride Health is actively hiring a Physical Therapist (PT) to join our client's growing Home Health team. Compensation: Local Pay: $55.00 - $70.00/hour (W-2) Travel Pay Package: Up to $2,800/week gross (includes weekly non-taxable stipend)* Location: Gallipolis, OH, 45631 Schedule: Monday-Friday | 8:00 AM - 4:30 PM Productivity expectations: Full time, 40hrs/week, 30 points On Call/Weekend Requirement: N Coverage Area: Jackson and Gallia counties primarily, rarely Meigs and Athens counties.

Key Responsibilities:

  • Conduct comprehensive home health physical therapy evaluations, OASIS assessments (SOC, ROC, Recertification, Discharge, and 30-day reassessments), develop individualized treatment plans, deliver skilled therapy interventions, and document care accurately while meeting a productivity expectation of 30 points per week across Jackson and Gallia counties.
  • Monitor patient progress, supervise PTA services, collaborate with the interdisciplinary care team to optimize functional outcomes, ensure compliance with Medicare and agency standards, and provide patient/caregiver education while maintaining timely documentation in HCHB/PointCare (preferred).

Must have qualifications/experience: Active PT license in Ohio, current CPR, current Driver's License, dependable vehicle, current vehicle insurance. What skills/attributes are nice to have, and will set a candidate apart? Home Health experience required. HCHB/Point care experience preferred. Type of certification required: Active PT License for Ohio Dress code/scrub colour: Grey scrubs with full coverage shoes. Scrubs should be fairly wrinkle free, no rips, tears or stains. You may wear a matching color undershirt and/or scrub jacket.
