Job Details:
Schedule: Day shift, 8 hours per day, with a minimum of 40 hours per week
Required Qualifications: Degree in Physical Therapy from an accredited school; 1 or more years of outpatient clinical experience; CPR certification; licensed or eligible for licensing in Indiana; excellent communication skills and ability to follow clinical and organizational protocols
Preferred Qualifications: Experience working in outpatient therapy services; familiarity with clinical reasoning and documentation standards; commitment to professional development
Behavior Characteristics: Demonstrates professionalism, accountability, teamwork, resourcefulness, and safety awareness in all aspects of patient care and work environment
Additional Information: Performs comprehensive patient evaluations, develops individualized treatment plans based on clinical reasoning, and documents care in accordance with state, federal, and organizational guidelines. Participates in student programs providing supervision and mentorship. Maintains licensure in good standing and pursues ongoing professional development. Adheres to all safety and organizational performance improvement initiatives.
