Minimum 6 months of recent experience within the last 6 months in Physical Therapy/Rehab Active PT license required if applicable No gaps in work history of 90 days or more within the past 12 months Additional Information
Candidates must reside at least 50 miles from the facility to be eligible; local candidates will not be considered One professional reference required for most states; Florida and Missouri require two references US Social Security Number and date of birth required for consideration; Canadian SSNs are not accepted
