Overview
We’re seeking an experienced Travel Physical Therapist for a Home Health assignment in Sonora, California . This role is ideal for PTs who are comfortable working independently and managing a home health caseload.

Setting: Home Health
Duration: 13 Weeks (extension possible)
Schedule: Full-time (based on patient census)
Requirements
- Minimum 2 years of Physical Therapy experience – Preferred
- Home Health experience – Required
- HCHB (Homecare Homebase) experience – Required
- OASIS documentation experience – Required
- BLS/CPR Certification – Required
- Valid Driver’s License – Required
Responsibilities
- Provide in-home physical therapy evaluations and treatments
- Complete OASIS assessments and documentation accurately
- Develop and implement individualized care plans
- Educate patients and caregivers on therapy plans and safety
- Collaborate with interdisciplinary home health teams
- Maintain compliance with state and federal home health regulations
