American Traveler is seeking a Home Health Physical Therapist with at least 1 year of relevant experience and proficiency in HCHB documentation.
Requirements
- Minimum of 1 year of relevant home health physical therapy experience
- OASIS experience required for staff completing Start of Care (SOC) documentation
- Proficiency in HCHB (Homecare Homebase) charting system required
- Active driver's license and personal transportation required
- License required if applicable
- Prior travel experience required; first-time travelers will not be considered
Responsibilities
- Home health setting within a PT Home Health unit
- Day shift schedule, 5x8 hours from 8:00 AM to 4:30 PM
- On-call rotation may be required; details to be discussed during interview
- Travel pairs accepted, though each traveler is assigned to their own territory and must have personal transportation
- Timely communication and documentation practices are mandatory
- Orientation begins on Mondays and includes completion of billable training modules
- Hospice/Home Health checklist completion is mandatory
- Requested time off (RTO) must be submitted at the time of application
- Former permanent employees must be separated for a minimum of 6 months and approved by HR before returning as a traveler
