Position: Home Health Physical Therapist - Portland, OR
Schedule: 8-hour days, minimum of 40 hours per week, starting on 09/07/2026
Required Qualifications: 1 year of skilled home health experience, applicable state licensure, BLS certification, and reliable transportation.
Preferred Qualifications: Experience with OASIS documentation and wound care skills.
Patient Ratios & Responsibilities: May visit up to 6 patients daily; clinician is expected to meet productivity standards and complete all documentation accurately using EPIC and Rover systems.
Certifications and Licenses: BLS required; Wound Care certification preferred.
Additional Information: Traveler is responsible for their own transportation; minimal travel radius of approximately 60 miles per day; must have completed the primary COVID-19 vaccination series plus a booster. The facility emphasizes a team-oriented approach with minimal windshield time.
System Experience: EPIC for charting; OASIS documentation experience is preferred.
