Responsibilities

• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans to restore and improve physical function
• Deliver therapy interventions using evidence-based techniques and established protocols
• Collaborate with an interdisciplinary team and participate as a co-leader in patient group activities
• Complete accurate clinical documentation within established deadlines; communicate discharge plans to the care team
• Supervise assigned assistants and provide patient education on safety and self-management
• Maintain EPIC electronic health record documentation and ensure continuity of care across transitions

Requirements

• Doctor of Physical Therapy (DPT) degree; required, if license was obtained on or after January 2016, or foreign equivalent
• Physical Therapy licensure in the state of practice (California) required
• CPR/BLS certification required
• At least two years of recent physical therapy experience
• Pelvic floor therapy experience with PRPC or WCS certification, or actively pursuing certification (preferred)
• EPIC experience required
• Current nursing license per the state of assignment (as referenced in job notes)
