Provide physical therapy services within a CMS Certified Home Health Agency setting Conduct Start of Care (SOC) evaluations and complete OASIS assessments accurately and in a timely manner Develop and implement individualized patient care plans based on evaluation findings Deliver skilled physical therapy interventions to patients in a home health environment Document patient progress and clinical notes utilizing electronic health record systems Required Qualifications
Active Physical Therapist (PT) license in the state of Washington Prior Home Health experience required Demonstrated experience with SOC evaluations and OASIS documentation Proficiency with HCHB (Homecare Homebase) electronic medical records system Preferred Experience / Skills
Strong familiarity with CMS Certified Home Health Agency workflows and compliance requirements Experience working within a high-volume home health caseload Why Join Us
