Physical Therapist (PT)
to join our outpatient rehabilitation team. The Physical Therapist will evaluate patients, develop individualized treatment plans, and provide evidence-based therapy services to adults with
brain injuries and other neurological diagnoses , helping them improve mobility, function, independence, and overall quality of life. The ideal candidate will have previous
outpatient experience , particularly treating adults with neurological conditions, and be committed to delivering high-quality, patient-centered care in a collaborative clinical environment. Schedule
Shift:
Monday – Friday, 8:00 AM – 5:00 PM Responsibilities
Perform comprehensive physical therapy evaluations and develop individualized treatment plans Provide evidence-based treatment for adults with brain injuries and other neurological conditions Implement therapeutic exercises, gait training, balance training, neuromuscular re-education, transfer training, and functional mobility interventions Monitor patient progress and modify treatment plans to maximize functional outcomes Educate patients and caregivers on home exercise programs, mobility techniques, fall prevention, and safety strategies Maintain accurate and timely documentation in accordance with facility policies and regulatory standards Collaborate with physicians, occupational therapists, speech-language pathologists, and other members of the interdisciplinary healthcare team Participate in discharge planning and provide recommendations for continued therapy services Maintain a safe, clean, and organized treatment environment Deliver compassionate, patient-centered care focused on improving function, mobility, and quality of life Requirements
Required:
Graduate of an accredited Physical Therapy program Required:
Active
Physical Therapist (PT)
license in the state of practice or ability to obtain Preferred:
Previous
outpatient experience
treating adults with
brain injuries and other neurological diagnoses Strong clinical reasoning, communication, documentation, and organizational skills Ability to work independently and collaboratively within a multidisciplinary healthcare team Commitment to providing exceptional, patient-centered care What We Offer
